  • HMSA (if your plan goes through ASH, there is an additional fee of $59.71. Call HMSA to check your plan.)

  • HMAA

  • Blue Cross Blue Shield

  • ASH * (through HMSA, Kaiser, or Cigna)

    *with a fee of $59.71 + your ASH copay

 

Please Note: If you have an HMO plan, you will need a referral from your Primary Care Provider.
